If you share your home with a Yorkshire Terrier, Pomeranian, Chihuahua, or Toy Poodle, you may already know the sound: a dry, harsh cough that veterinarians and owners alike describe as a "goose honk." It flares up when your dog gets excited, pulls on the leash, drinks water, or on hot, humid days. That distinctive honk is the signature of tracheal collapse — a chronic, progressive weakening of the windpipe that is one of the most common causes of coughing in small and toy breeds. It can't be cured, but with the right management most affected dogs live long, comfortable, happily noisy lives.

What Is Tracheal Collapse?

The trachea, or windpipe, is a flexible tube held open by 35 to 45 C-shaped rings of cartilage. The open part of the "C" faces upward and is spanned by a soft band of tissue called the dorsal tracheal membrane. In a healthy dog, the cartilage rings are rigid enough to keep the airway round through every breath.

In dogs with tracheal collapse, the cartilage rings gradually lose their stiffness. According to the Merck Veterinary Manual, the weakened rings flatten from top to bottom, and the slack dorsal membrane droops into the airway. The result is a windpipe that narrows or closes off during breathing — like a paper straw that collapses when you sip too hard. The obstruction irritates the airway lining, triggering the cough, and the coughing itself inflames the airway further, creating a frustrating cycle.

Veterinarians grade the severity from Grade 1 (the airway loses about 25% of its diameter) through Grade 4 (near-complete collapse). The collapse can occur in the neck, inside the chest, or both, and the location influences whether the trouble comes when breathing in or breathing out.

Which Dogs Are at Risk

Tracheal collapse is overwhelmingly a small-dog condition. Yorkshire Terriers account for roughly two-thirds of reported cases, with Pomeranians, Chihuahuas, Toy and Miniature Poodles, Shih Tzus, and Pugs also frequently affected. The disease is rare in large breeds and in cats. Most dogs begin showing signs in middle age — typically between 4 and 8 years old — although some are affected while young and others not until their senior years.

The underlying cause is thought to be a congenital weakness in the cartilage itself: affected dogs have rings with less glycosaminoglycan and calcium, which makes them softer than normal. But several factors determine whether that weakness produces symptoms, and how bad they get:

The Signs to Watch For

The classic sign is the honking cough — dry, harsh, and often coming in fits. It is typically triggered by excitement, exercise, eating or drinking, pressure on the throat, or hot weather, and it often gets worse at night. Many dogs also gag or retch at the end of a coughing fit.

As the disease progresses, some dogs develop signs that go beyond the cough: noisy or wheezy breathing, exercise intolerance, and in severe episodes, labored breathing, blue-tinged gums, or collapse. Those last three are emergencies. A dog that cannot catch its breath and is turning blue needs a veterinary hospital immediately — severe episodes can be fatal without oxygen and sedation to break the cycle of panic and airway obstruction.

Because the honking cough resembles kennel cough, heart disease, and chronic bronchitis, diagnosis matters. Your vet may use chest X-rays, fluoroscopy (a moving X-ray that catches the trachea collapsing in real time), or tracheoscopy — passing a small camera into the airway under anesthesia — to confirm the diagnosis and grade its severity.

Treatment: Medical Management Comes First

The encouraging news, per the American College of Veterinary Surgeons, is that medical management controls signs in about 70% of dogs, especially those with mild to moderate collapse. Treatment usually combines medication with lifestyle changes:

When Medicine Isn't Enough: Rings and Stents

For dogs with severe collapse that no longer respond to medication, two interventions can restore an open airway. For collapse in the neck, a board-certified surgeon can place prosthetic rings around the outside of the trachea to hold it open. For collapse inside the chest — or dogs who aren't surgical candidates — an interventional specialist can place a self-expanding nitinol stent inside the airway through the mouth, with no incision at all.

Both procedures can be life-changing for the right patient, but they are salvage procedures, not cures: coughing usually persists to some degree, ongoing medication is still needed, and complications like stent fracture or granulation tissue are possible. Published outcomes report average survival of roughly two to two-and-a-half years after stenting, with many dogs doing considerably better. These cases belong in the hands of specialty centers, and your regular vet can refer you when the time is right.

Living Well With a Collapsing Trachea

A diagnosis of tracheal collapse sounds frightening, but for most dogs it's a condition to be managed, not a countdown. Keep your dog lean, walk with a harness, banish smoke from the house, avoid the midday heat, and use the medications your vet prescribes at the first sign of a flare-up.
