Of all the intestinal parasites that affect dogs and cats, hookworms may be the most underestimated. They're small — barely a half-inch long — and unlike tapeworm segments or adult roundworms, you'll almost never see one in your pet's stool. But hookworms earn their name from the hook-like mouthparts they use to latch onto the lining of the small intestine, where they do something the other common worms don't: they drink blood.

In an adult dog or cat, a light hookworm infection may cause nothing more than mild digestive upset. In a young puppy, a heavy infection can cause severe, even fatal, anemia in a matter of weeks. And because hookworm larvae can burrow through human skin, these parasites are a public-health concern too. Here's what every pet owner should know.

What Hookworms Are and How Pets Get Them

Several hookworm species infect companion animals. Ancylostoma caninum is the most important species in dogs and an aggressive blood-feeder; Ancylostoma tubaeforme is its counterpart in cats; and Ancylostoma braziliense and Uncinaria stenocephala (a cooler-climate species) can infect both. Adult worms live in the small intestine, attach to the intestinal wall, feed, and shed microscopic eggs into the feces. In warm, moist soil those eggs hatch into infective larvae within about a week.

Because larvae thrive in warm, damp conditions, hookworms are found nearly worldwide but are especially common in warm and humid regions. Dog parks, kennels, beaches, and any yard where feces are left on the ground can harbor larvae for weeks.

Signs of Hookworm Infection

Many infected adult pets show no obvious symptoms, which is exactly why routine fecal testing matters. When signs do appear, they reflect blood loss and intestinal irritation:

Puppies are by far the most vulnerable. Veterinary parasitologists describe A. caninum as a voracious bloodsucker, and a heavy burden can drain a young puppy faster than its body can make new red blood cells. Severe cases can require emergency care, including blood transfusion. Cats generally develop milder disease — the feline hookworm takes less blood — but kittens and debilitated cats can still become anemic.

Diagnosis: Why One Negative Test Isn't Always the End

Hookworms are diagnosed with a fecal flotation test, in which a stool sample is examined under a microscope for the parasite's distinctive oval eggs. There's an important catch: it takes roughly two to three weeks after infection before adult worms begin shedding eggs. A very young puppy infected through nursing can be seriously ill before any eggs appear in its stool. That's why veterinarians treat puppies presumptively and may recommend repeat fecal exams — the Companion Animal Parasite Council recommends testing puppies and kittens multiple times during their first year and adult pets at least once or twice yearly.

Treatment

The good news: hookworm infections are very treatable. Common dewormers approved for hookworms include pyrantel pamoate, fenbendazole, moxidectin, and milbemycin oxime — several of which are already ingredients in monthly heartworm preventives. Because these drugs kill adult worms but not migrating larvae, treatment is typically repeated after two to three weeks to catch worms that have matured in the interim. Pets with significant blood loss may also need iron supplementation, a high-protein diet, or in severe cases a transfusion.

Two follow-up points are worth knowing. First, in dogs, dormant larvae in body tissues can periodically "leak" back into the intestine — a phenomenon called larval leak — so a previously treated dog can test positive again without being reinfected from outside. Second, veterinary parasitologists have documented multi-drug-resistant strains of A. caninum in the United States in recent years, originally traced to racing greyhound populations. If your dog has a hookworm infection that keeps coming back despite proper treatment, tell your veterinarian — follow-up fecal testing after deworming is the way to confirm the treatment actually worked.

Hookworms Can Infect People, Too

Hookworms are zoonotic. When people walk barefoot, sit, or kneel on soil or sand contaminated with larvae, the larvae can penetrate human skin. In people they usually can't complete their life cycle; instead they wander in the upper layers of skin, producing cutaneous larva migrans — intensely itchy, red, winding tracks that creep across the skin, most often on the feet. According to the CDC, this is the most common manifestation of zoonotic hookworm infection, and it's treatable with antiparasitic medication. Children playing in contaminated sandboxes and anyone walking barefoot where dogs defecate are at highest risk — one more reason picking up after pets is a public service, not just good manners.

Hookworm prevention is inexpensive and fits into routines most pet owners already have:

Hookworms are a nearly invisible parasite with a very visible worst-case scenario — but between modern preventives, routine fecal testing, and basic yard hygiene, they're also one of the easiest problems to keep out of your pet's life entirely.
