Most eye problems in dogs allow you a day or two to watch and wait. Glaucoma is the exception. When pressure inside the eye spikes, the retina and optic nerve begin dying within hours, and the damage does not come back. Veterinary ophthalmologists consistently rank glaucoma among the leading causes of irreversible blindness in dogs — and the difference between a dog that keeps some vision and a dog that loses the eye often comes down to how fast the owner recognized that a red, cloudy, squinting eye was not "just conjunctivitis."

What Glaucoma Actually Is

The front chamber of the eye is constantly filled and drained by a clear fluid called aqueous humor. It is produced behind the iris, flows through the pupil, and exits through a mesh-like drain called the iridocorneal angle, where the iris meets the cornea. In a healthy dog this inflow and outflow balance to a pressure of roughly 10 to 25 mmHg, according to the Merck Veterinary Manual.

In glaucoma, the drain fails. Fluid keeps being produced but cannot leave, so pressure climbs — sometimes to 40, 50, or even beyond 60 mmHg. That pressure crushes the two structures that make vision possible: the retina, which captures light, and the optic nerve, which carries the image to the brain. Neither regenerates. This is why glaucoma is fundamentally different from cataracts, where blindness can often be surgically reversed. Pressure-induced damage is permanent, and it accumulates by the hour.

Glaucoma is also genuinely painful. The pressure is comparable to a severe migraine, though dogs rarely show it the way we would expect. Many just sleep more, go quiet, or become head-shy — and many owners only realize how much pain their dog was in after the problem is treated and the dog visibly brightens.

Primary vs. Secondary Glaucoma

Primary glaucoma is inherited. The dog is born with a malformed or narrow drainage angle, and at some point in mid-life — typically between roughly 4 and 10 years of age — the drain closes and pressure spikes, usually in one eye first. Critically, primary glaucoma is a bilateral disease: the other eye has the same faulty anatomy and is very likely to follow.

Secondary glaucoma is caused by another problem physically blocking the drain: a lens that has slipped out of position (lens luxation), inflammation inside the eye (uveitis), an advanced cataract, bleeding, trauma, or a tumor. Research from the University of Florida analyzing the North American Veterinary Medical Data Base found that primary and secondary glaucomas together affect roughly 1.7% of dogs in North America — a strikingly high figure for a blinding disease, and in some breeds the prevalence runs several times higher.

The Breeds at Highest Risk

Primary glaucoma is strongly breed-linked. In the Gelatt and MacKay prevalence study, the breeds at the top of the list included the American Cocker Spaniel (5.5%) and Basset Hound (5.4%), followed by the Chow Chow, Shar-Pei, Boston Terrier, Wire Fox Terrier, Norwegian Elkhound, Siberian Husky, Cairn Terrier, and Miniature Poodle. Other commonly cited at-risk breeds include the Beagle, Samoyed, Jack Russell Terrier, and Great Dane.

If you own one of these breeds, two things are worth doing. First, ask your veterinarian to measure eye pressure at routine checkups once your dog reaches middle age — the test takes seconds. Second, memorize the warning signs below, because primary glaucoma tends to arrive suddenly, often in the evening or overnight.

Signs of an Acute Attack

The dangerous trap is that early glaucoma mimics milder conditions. A slightly red, watery, squinting eye looks a lot like conjunctivitis — but conjunctivitis doesn't cloud the cornea, blow the pupil, or hurt this much. Any red eye combined with a hazy cornea or a dilated pupil deserves a same-day pressure check.

Why Hours Matter

The Merck Veterinary Manual classifies acute glaucoma as a true ophthalmic emergency: sustained pressure above 40 to 50 mmHg can destroy retinal cells and the optic nerve within hours to a couple of days. Veterinary ophthalmologists' guidance is blunt — if you cannot be seen by your regular vet the same day, go to an emergency clinic. A pressure reading with a tonometer takes less than a minute, needs only a drop of local anesthetic, and settles the question immediately.

Emergency treatment aims to crash the pressure fast. Depending on the case, that may involve prostaglandin analog drops such as latanoprost — which can pull pressure down dramatically within an hour in primary glaucoma — topical carbonic anhydrase inhibitors like dorzolamide to slow fluid production, and in some cases intravenous or oral osmotic agents such as mannitol or glycerin that draw fluid out of the eye. Pain relief matters too; these dogs hurt.

Long-Term Treatment: Managing, Not Curing

Once pressure is controlled, the honest conversation begins. Medical therapy — usually a combination of pressure-lowering drops given for life — can hold glaucoma at bay, but in primary glaucoma the drain continues to deteriorate, and most medically managed eyes eventually escape control. The prognosis for keeping long-term vision in a primary glaucoma eye is guarded even with excellent care.

For eyes that still have vision, surgical options can buy significant time: laser cyclophotocoagulation reduces the fluid-producing tissue, and drainage implants (shunts) give the fluid an artificial exit; the two are often combined. For eyes that are already permanently blind, the priority shifts entirely to comfort. A blind, painful, pressurized eye serves no purpose, and options such as enucleation (removal of the eye) or an intrascleral prosthesis end the pain for good. Owners are often shocked by how much younger their dog acts after a blind, aching eye is removed — dogs adapt to one-sided vision remarkably well.

Protecting the Other Eye

If your dog is diagnosed with primary glaucoma, the most important question is not about the affected eye — it's about the other one. In a multicenter clinical trial published in the Journal of the American Animal Hospital Association, untreated fellow eyes developed glaucoma after a median of just 8 months. With prophylactic pressure-lowering drops, that median stretched to 31 months — roughly two extra years of vision from a once- or twice-daily eye drop. Regular pressure rechecks of the second eye are part of the deal, so rising pressure is caught before another acute, vision-destroying spike.

The Bottom Line

Glaucoma asks one thing of dog owners: speed. You cannot prevent primary glaucoma, and you cannot reverse the damage it does — but you can recognize a red, cloudy, painful, dilated eye for what it is and treat it like the emergency it is. If your dog is a Cocker Spaniel, Basset Hound, or another at-risk breed, put eye pressure checks on the annual checkup list and know where your nearest emergency clinic is. When glaucoma strikes, the dogs that keep their sight are the ones whose owners drove in that night, not the following week.
