It usually starts at three in the morning. A cat who spent fifteen years sleeping quietly at the foot of the bed now stands in the hallway, howling at nothing in particular. Owners try closing doors, adding night lights, feeding a late snack. Most assume it is simply what old cats do.
It often is not. Feline cognitive dysfunction syndrome — cat dementia — is a real, progressive neurodegenerative disease, and it is far more common than its low diagnosis rate suggests. Research screening older cats found signs of cognitive dysfunction in roughly 28 percent of cats aged 11 to 14, rising to more than 50 percent of cats aged 15 and over. Yet most affected cats are never formally diagnosed, because the changes arrive slowly and get filed under "just getting old."
What Is Actually Happening in the Brain
The aging cat brain undergoes measurable physical change: cortical atrophy, widening of the ventricles, neuron loss, and the accumulation of two abnormal proteins that will be familiar to anyone who has read about human dementia — beta-amyloid plaques and hyperphosphorylated tau. Oxidative damage from free radicals and reduced blood flow to the brain compound the injury over years.
How closely this mirrors human disease became clearer in 2025. A team led by researchers at the University of Edinburgh and the UK Dementia Research Institute examined the brains of 25 cats of varying ages after death and published the findings in the European Journal of Neuroscience. They found toxic beta-amyloid building up inside the synapses — the junctions where brain cells communicate — of older cats and cats with dementia. They also found astrocytes and microglia, the brain's support cells, engulfing those damaged synapses.
That matters because synapse loss is the single strongest predictor of cognitive decline in human Alzheimer's disease. The same mechanism appears to be at work in cats, which is why aged cats are now being studied as a naturally occurring model of Alzheimer's. For owners, the practical takeaway is simpler: this is a disease of the brain's physical architecture, not a personality change or a behavior problem your cat has chosen.
The Signs: What to Watch For
Veterinary behaviorists have long used the acronym DISHAA to organize the signs of cognitive dysfunction — Disorientation, altered Interactions, Sleep-wake cycle changes, House-soiling, Activity changes, and Anxiety. Cats do not present identically to dogs, so some researchers have proposed feline-specific frameworks, but the six categories remain a useful checklist.
- Disorientation. Standing in corners, staring at walls or into empty space, getting stuck behind furniture, appearing lost in a familiar room, or forgetting where the food bowl lives.
- Altered interactions. Some cats become clingy and demanding of attention; others withdraw from people and housemates they previously sought out. Irritability toward other pets is common.
- Sleep-wake disruption. Sleeping more during the day and becoming restless and vocal at night. Night yowling is frequently the first sign owners report — not because it comes first, but because it is the one that wakes them up.
- House-soiling. Urinating or defecating outside the litter box in a previously reliable cat, sometimes right beside it.
- Activity changes. Loss of interest in play, reduced grooming and a scruffy coat, aimless pacing or wandering, or repetitive behaviors.
- Anxiety. New fearfulness, clinginess, agitation, or distress when left alone.
Appetite changes cut both ways: some cats forget to eat and lose weight, while others seem to forget they have already eaten and beg repeatedly.
Why Diagnosis Means Ruling Things Out First
There is no blood test or scan that confirms feline cognitive dysfunction in a living cat. It is a diagnosis of exclusion, which makes the veterinary workup essential rather than optional — because several treatable conditions produce nearly identical signs in a senior cat.
- Hyperthyroidism causes restlessness, night vocalizing, weight loss, and behavior change. It is extremely common in older cats and very treatable.
- Systemic hypertension can cause disorientation, vocalizing, and sudden blindness from retinal detachment — a cat bumping into furniture may have a blood pressure problem, not a memory problem.
- Chronic kidney disease and diabetes both alter drinking, urination, and demeanor, and both can push a cat out of the litter box.
- Osteoarthritis is present in the majority of senior cats and is badly underdiagnosed. A cat who can no longer climb comfortably into a high-sided box will soil beside it, and a cat in pain may become withdrawn and stop grooming.
- Hearing and vision loss produce louder vocalizing and apparent confusion without any cognitive disease at all.
- Brain tumors, most often meningiomas in older cats, can mimic dementia closely.
- Pain, urinary tract disease, and hypertension-related pain also drive house-soiling and irritability.
Expect your veterinarian to run a full physical and neurological exam, complete blood count and chemistry panel, total T4, urinalysis, and blood pressure measurement. Bring video. A thirty-second phone clip of your cat at 3 a.m. tells a veterinarian more than any description you can give in an exam room.
What Actually Helps
There is no cure and no way to reverse neuron loss. But cognitive dysfunction is manageable, and cats often respond better than owners expect — particularly when intervention starts early. The evidence supports a combined approach rather than any single fix.
Environmental enrichment and routine. This is the most consistently supported intervention. Mental stimulation promotes neuron survival and measurably improves cognition in aging animals. Short, gentle play sessions, food puzzles adapted to an older cat's ability, scent enrichment, and predictable daily routines all help. The key is adapted enrichment: a puzzle a senior cat cannot solve becomes a source of frustration, not stimulation.
Make the environment forgiving. Add extra litter boxes with low entry sides on every floor the cat uses. Keep food, water, and resting spots close together and never move them. Use ramps or steps to favorite perches. Leave night lights on along the routes your cat travels after dark. Avoid rearranging furniture — a cat with a failing spatial map depends on the one it already has.
Diet and supplements. Diets enriched with antioxidants, essential fatty acids, and medium-chain triglycerides have shown cognitive benefit in aging pets. Supplements used in cats include SAMe (S-adenosylmethionine), and feline-formulated antioxidant blends such as Senilife and Aktivait for Cats. Discuss any supplement with your veterinarian first — Aktivait formulated for dogs contains alpha-lipoic acid and must never be given to cats, which are sensitive to it.
Medication. No drug is licensed specifically for feline cognitive dysfunction in most countries, but selegiline and propentofylline are prescribed off-label by some veterinarians, and anxiolytics or gabapentin may help cats whose night-time distress is severe. Improvement, when it comes, typically takes several weeks.
Pheromones. Synthetic feline facial pheromone diffusers are low-risk and may reduce anxiety and night-time agitation in some cats.
Living With a Cat Who Is Losing Their Map
Cognitive dysfunction is progressive, and the honest framing is that treatment slows decline and protects quality of life rather than restoring what has been lost. That is still worth a great deal. Many cats live comfortably for years after diagnosis with nothing more than an adapted home, a steady routine, and good pain control.
Two things make the biggest difference. The first is catching it early, which means twice-yearly veterinary visits after age 10 and mentioning behavior changes rather than waiting to be asked. The second is treating the whole cat: arthritis pain, dental disease, and hypertension all worsen cognitive signs, and addressing them often produces more visible improvement than anything aimed at the brain directly.
Above all, resist the temptation to punish. A cat urinating on the rug at fifteen is not being spiteful or lazy — they have lost part of the internal map that told them where to go. Scolding adds anxiety to a brain already struggling with it. What helps is patience, predictability, and a home rearranged around the cat your cat has become.
