One day your dog jumps off the couch and comes up limping. The next morning the limp is better, then worse, then better again. A week later you notice they won't put full weight on a back leg, the thigh looks a little thinner, and they hesitate before climbing the stairs. There is a good chance you are watching a cranial cruciate ligament — the canine version of the human ACL — slowly fail.

Cranial cruciate ligament (CCL) rupture is the single most common cause of pelvic-limb lameness in dogs and one of the most studied orthopedic conditions in veterinary medicine. Unlike the human ACL injury, which is usually a single dramatic tear during sport, the canine version is mostly a slow degenerative process — which is why understanding the early signs matters.

What the CCL Actually Does

The cranial cruciate ligament sits inside the stifle (the dog's knee) and stops the tibia from sliding forward relative to the femur during weight bearing. It also limits hyperextension and internal rotation. When the ligament is intact, the joint is stable enough that the muscles, menisci, and cartilage can do their jobs efficiently. When the CCL begins to fray or fully tears, the tibia thrusts forward with every step, the joint becomes unstable, and the medial meniscus — a wedge-shaped shock absorber between the bones — is at risk of being crushed and torn.

It's Usually Degenerative, Not a Sports Injury

This is the most important concept for owners to understand. In humans, the ACL typically ruptures during a single high-energy event — a sudden pivot in basketball, a hard tackle in soccer. In dogs, that is the exception rather than the rule. Most canine CCL ruptures are the end stage of a chronic degenerative process called cranial cruciate ligament disease, in which the collagen fibers within the ligament gradually weaken over months or years until even a routine movement is enough to finish them off.

That is also why so many dogs eventually rupture both knees. The underlying disease process affects both ligaments at once; one simply fails first. Studies have repeatedly shown that roughly 40–60% of dogs with a unilateral CCL rupture will rupture the contralateral ligament, often within one to two years.

Which Dogs Are Most at Risk

CCL disease can affect any dog, but several factors meaningfully increase risk.

Because the disease is usually progressive, the signs often start subtle and intermittent before anything dramatic happens. Common patterns include:

If a dog suddenly cannot weight-bear at all, or you hear a click in the joint when they walk, that is a strong hint that the meniscus has been damaged as well — a more painful problem that should not wait.

How Vets Diagnose CCL Rupture

Diagnosis combines history, hands-on orthopedic exam, and imaging. The two classic stability tests are the cranial drawer test and the tibial compression (thrust) test. In the drawer test, the vet stabilizes the femur and tries to slide the tibia forward; abnormal forward movement indicates ligament failure. In the tibial thrust test, the vet mimics weight bearing and feels for the same forward tibial movement. Both tests are more reliable under sedation, because awake dogs often tense their muscles and mask instability — particularly in partial tears, where the joint may only feel unstable in flexion.

Radiographs (X-rays) cannot show the ligament itself, but they reveal the secondary changes that almost always accompany CCL disease: joint effusion, cranial displacement of the tibia, and osteophytes (bony spurs) from osteoarthritis. They also rule out other causes of lameness, including bone tumors in older large-breed dogs. Advanced imaging — MRI, CT, or arthroscopy — is sometimes used for ambiguous cases or to assess the menisci before surgery.

Treatment: TPLO, TTA, Lateral Suture, and Conservative Management

No single treatment is right for every dog, but the evidence is reasonably consistent for the general categories.

Tibial Plateau Leveling Osteotomy (TPLO). The current standard of care for most medium, large, giant, and athletic dogs. Rather than replacing the ligament, the surgeon cuts and rotates the top of the tibia so that the joint no longer needs the CCL to stay stable during weight bearing. Long-term studies — including the Christopher et al. comparison published in Veterinary Surgery in 2013 — show superior limb function and owner satisfaction with TPLO compared with lateral suture techniques, and arthritis tends to progress more slowly. Most dogs are back to controlled walking within two weeks and to normal activity by four to six months.

Tibial Tuberosity Advancement (TTA). Another osteotomy procedure that changes the geometry of the stifle so the CCL is no longer needed for stability. Meta-analyses have found broadly similar long-term outcomes for TPLO and TTA in appropriately selected patients, with some differences in complication profiles. The choice often comes down to surgeon preference and a dog's specific anatomy.

Lateral suture (extracapsular repair). A heavy suture placed outside the joint capsule that acts as a temporary stabilizer while scar tissue forms. Generally reserved for smaller, less active dogs. In medium and large dogs, head-to-head studies have favored TPLO over lateral suture for long-term function.

Conservative (non-surgical) management. Reasonable for some small dogs (typically under about 10 kg), for owners for whom surgery is not financially feasible, or for dogs who are poor anesthetic candidates. It usually combines strict activity restriction, weight loss, physical rehabilitation, joint supplements, and pain control. A 2024 study in small-breed dogs under 10 kg found acceptable outcomes for many — though function still trailed surgically treated dogs at long follow-up. For most medium and large dogs, surgery clearly produces better long-term limb use and slower arthritis progression.

Recovery and Rehabilitation

The surgery is only the first half of the story. Outcomes are strongly tied to the quality of postoperative care. A typical recovery looks like 8 weeks of strict activity restriction (leash walks only, no jumping, no off-leash running, no stairs when avoidable), followed by a gradual return to normal activity over the next 2–4 months. Formal physical rehabilitation — controlled passive range of motion, underwater treadmill, targeted strengthening — speeds the return to function and improves long-term outcomes, particularly in larger or athletic dogs. Owners who skip rehab and let dogs decide their own activity level tend to see slower, less complete recoveries.

Protecting the Other Knee

Because so many dogs go on to tear the second CCL, every dog recovering from one rupture is also a candidate for prevention on the other side. The interventions are not exotic — but they matter.

The Bottom Line

CCL rupture in dogs is common, treatable, and very rarely a single bad-luck event. The earlier it is recognized, the more options you have — and the better the chances of protecting the other knee. If your dog is intermittently limping on a back leg, sitting with one leg kicked to the side, or hesitating before stairs, do not wait for it to "walk itself off." A focused orthopedic exam with your veterinarian, or a referral to a board-certified surgeon, can give you a clear picture of what is happening and a plan that gets your dog back to comfortable, confident movement.
